Submitting a Change of Name Petition Last Updated:  Apr 02, 2007, 1:34 PM PDT

The university maintains your records under your full legal name.

The following documentation is required to change your name:*

Name currently on UCI records
• documentation showing your name as it currently appears on your UCI records (picture ID preferred; birth certificate or Social Security card is acceptable). Strong evidence is required to establish a link between you and the record being changed.

AND

One of the following pertaining to your personal circumstances:

Name change by marriage
• copy of marriage certificate, OR license and certificate of marriage, OR license and certificate of confidential marriage, OR certified abstract of marriage

Name change by dissolution of marriage
• copy of dissolution of marriage judgement with maiden name restored

Name change by court order
• copy of court order Name change by naturalization
• copy of naturalization certificate with the Application for Change of Name

Name change by usage method
• documentation demonstrating you are using your new name to transact business with state and federal agencies (both are required):

• valid California driver's license or California ID card with new name
• Social Security card with new name
• documentation that "links" your current and new name

*Changing your name on the UCI record will not change your name on your diploma. Log in to StudentAccess and make sure your name is correct on the Application for Graduation.


Submit your completed Change of Name Petition and all required documentation to the Registrar's Office:
  • In Person: 215 Aldrich Hall (building 111 on a campus map)
  • By Mail:

Change of name petitions are processed as they are received.

Your student number will remain the same.
